A form such as this is a simple way to document the training of various employees on the contents of a particular SOP or other document. This log page can be attached to a copy of the applicable document and circulated to the appropriate employees to read. In many cases a trainer will need to provide additional explanation and/or demonstrations; the employees may be also be required to execute the procedure under supervision of the trainer.  Refresher training may be documented in the same log. If the document is revised, a new log page must be issued for the new version, and training on the new version must be completed.
