Employee Proficiency Testing Log

A form such as this can be used to document employee proficiency testing. Whereas some procedures are simple enough to understand merely from reading the applicable SOP, others will likely require new operators to perform the procedure several times under the close supervision of a trainer. Examples might include the disassembly of a machine or calibration of a measuring device. In such cases, formally documented proficiency testing may be appropriate. The employee should not be considered properly trained until he or she can successfully execute the procedure several times without help. A log such as the one below provides a way to track the progress of the employee’s training on each such procedure, which may extend over a period of days, weeks, or even months.
